Empactful Capital led the Series A financing for Care Continuity, a leader in patient navigation and care transitions.

In conjunction with the Series A, three new board members were announced including, Anthony V. Coletta, MD, MBA (EC Venture Partner), Yvette Bright (EC Special Advisor), and Sal DeTrane (EC Managing Director). Empactful also has a Board Observer seat to support the growth plan which has been filled by Doug Williams (EC Venture Partner).
